Youth Ministry Registration

1. Follow Youth Minister or Adult Leader instructions at all times;
2. Dress appropriately;
3. All participants must login under their real name. Anybody logging in under an assumed name will be dismissed and denied further access.
4. Refrain from behavior or using language that is inappropriate. This includes but is not limited to:
   1. bad language;
   2. presentation of inappropriate images or music;
   3. visible use or possession of alcohol, illegal drugs, or weapons;
   4. disruptive actions that are tantamount to bullying, harassment, or verbal abuse.

I hereby grant permission for my child to be photographed and/or videotaped during church activities and events. I understand that my child may decline to be photographed and/or videotaped at any time. I further grant permission for the resulting photographs and/or videotaped footage to be edited, if necessary, and then published and/or broadcast for the purpose of promoting youth programs at St. Louise Parish.
Yo permito que mi hijo sea fotografiado  y / o grabado en video durante las actividades y eventos de la iglesia. Entiendo que mi hijo puede negarse a ser fotografiado y / o grabado en video en cualquier momento. También doy permiso para que las fotografías y / o grabación de video resultantes sean editadas, si es necesario, y publicadas o transmitidas con el propósito de promover programas de la juventud en la parroquia de Sta. Luisa.

I hereby decline to grant permission for my child to be photographed and/or videotaped during church activities and events. I have instructed my child to decline to be photographed and/or videotaped at all times. I have further instructed my child to notify their Youth Minister and/or Core Team members that he/she may not be photographed and/or videotaped under any circumstances.
Yo NO permito que mi hijo sea fotografiado  y / o grabado en video durante las actividades y eventos de la iglesia. He instruido a mi hijo (a) para que rehace ser fotografiado y / o grabado en video en todos momentos. Además, he instruido a mi hijo a notificar a su Ministro de la Juventud y / o a los miembros del equipo que él / ella no puede ser fotografiado y / o grabado en video bajo ninguna circunstancia.
